Application/Plan Submittal Form

Please provide the name and contact information for the person whom City staff may contact for questions and/or comments regarding this submittal ("Project Applicant"). A copy of this completed submittal form will be sent to the email address filled out in the Contact Email field.

Please note that email will be the primary form of communication regarding this submittal.

New Project/Permit: If this is a new application and you are not already working with a specific person, please select New Project/Permit: Next Available Planner, and your application will be routed to the appropriate staff member.

Resubmittal: If you are already working with a specific City staff member, please select their name from the list below. 

Wireless Telecommunication Project/Permit Submittal: If you are submitting a wireless telecommunication project/permit, please select Wireless Telecommunication Project/Permit. Wireless telecommunication project/permit submittals are only accepted on Mondays and Tuesdays from 8 a.m. to 4 p.m. In-person wireless telecommunication project/permit submittals are by appointment only, and walk-in applicants will not be accepted. Appointments can be scheduled by calling (562) 916-1201. Any in-person or online wireless telecommunication project/permit submitted after 4 p.m. on Monday, whether online or in-person, will be deemed submitted on the following Tuesday, and any application submitted on Tuesday after 4 p.m. will be deemed submitted on the following Monday (or the following Tuesday if Monday is a legal holiday).

File Upload

Please upload all required application forms and supplemental files below. Supplemental files may include required site plans, architectural building elevations, photographs, and other supplemental information specified in the related application form. Photographs may be submitted in JPEG, PNG, or PDF. All other plans and documents must be submitted in PDF.
